By providing my signature, I confirm that as a participating organization, I agree to abide by the Terms and Conditions outlined in the Participating Organization and Donor Handbook including, but not limited to, the following:

Organization will provide documentation and EIN (tax id) number confirming 501(c)(3) status
Donors will not be reimbursed in any manner for any and all donations
Self-donations (donations made by a participating organization to themselves and paid by themselves) are prohibited
All matching funds generated must be used in the Tampa Bay area

Failure to abide by these rules and regulations will result in all monies being returned to the participating non-profit organization with no matching contribution.
